Assisted Living

Chosen Valley Assisted Living Apartments opened in 2006 with 20 apartment units available to seniors. Apartments are located on upper and lower levels with an elevator for convenience. Chosen Valley Assisted Living includes four large one bedroom, twelve small one bedroom and four studio deluxe apartments. Each unit includes a handicap design. There are comfortable and spacious common areas, as well as an outdoor patio and deck. Apartments are rented on a monthly basis.

Building security is provided through a locked front door. An intercom system in each apartment allows communication with visitors and the front door may then be unlocked from within the apartment.

The monthly fee includes the following services:

Home-style meals served daily in the Dining Room
24 hour/day personal care staff on duty for emergency needs
Weekly light housekeeping service
Weekly laundry service for flat linens
Daily “Wellness” Check
Bathing assistance once per week
Scheduled social, recreational and educational programs
Assessment by RN upon move-in and as health status changes are observed
Monthly blood pressure clinic
Utilities including: heat, electric, water, sewer and trash

Rooms

We want you to have the most comfortable stay as possible. Our assisted living apartments offer a choice of studio deluxe or one bedroom apartments. Each unit has individual climate controls, bathroom with walk-in shower and grab bars, walk-in closets, as well as a kitchen with sink, refrigerator & microwave oven. Apartment units are of handicap design, with an intercom system allowing communication with visitors from the secure main entrance door. Common Areas

Chosen Valley Assisted Living offers relaxing and spacious common areas for everyone to enjoy. Tenants will find a library room, a gathering room with tables, exercise bikes and large screen television, as well as an outdoor patio on the lower level. The upper level includes a living room area with television and an elegant dining room that adjoins a beautiful outdoor deck. Both levels offer fully equipped kitchens which tenants, families and friends can use to prepare a favorite dish. We do our very best to ensure our common living areas meet everyone’s needs.
